- Completing the opt‑in form below;
- Physicians and staff: providing your mobile number as part of your professional contact information to receive urgent patient‑callback and on‑call coverage alerts.

- Frequency: message frequency varies based on your appointments and care activity.
- Cost: message and data rates may apply, per your mobile carrier plan.
- HELP: reply HELP at any time for assistance, or call us at (909) 542‑2777.
- STOP: reply STOP at any time to unsubscribe. You will receive one confirmation message and then no further texts.
- Carriers are not liable for delayed or undelivered messages.

We respect your privacy. For this text‑messaging program:
- We collect only the information needed to send these messages — your name and mobile number, and the appointment/care details relevant to the message.
